Health and Wellness Coach
Roane General Hospital in Spencer, WV, a Critical Access Hospital with 35 SNF/ICF beds, 25 acute care beds, Level IV Trauma Emergency Department, three Rural Health Clinics and a walk-in clinic, is currently seeking applicants for a full-time Health and Wellness Coach.
Description:
The health and wellness coach will support the pillars of the hospital’s community wellness program, Prescription for Your Health, by coaching and directing participants. This individual will focus on health coaching and the integration of that coaching with the pillars of the Prescription For Your Health program. Those pillars include: community education and nutrition, medical fitness, and the medical plan of care with the patient’s medical providers. The coach will develop and assist with initiatives that will enhance the growth of the Center for Wellness and Prescription for Your Health for members, outreach programs, and employees. The health coach will coordinate the enrollment process and the collection of participant demographic data. The health coach will have the opportunity to work with patients and providers as a member of the multidisciplinary team to deliver high-quality care for our patients in the wellness center.
The health coach will be responsible for coordinating clients participating in Roane General Hospital Prescription for Your Health (P4YH), providing health coaching per best practices. The health coach will meet with patients / clients to develop comprehensive integrated wellbeing care plans and refer clients needing more intensive management to the appropriate case management, provider, community resource, dietitian, diabetic educator, or complex needs health coach. The health coach acts as a liaison between the patient, treating physician(s), and P4YH staff utilizing a multi-disciplinary approach to assess, identify, and plan health goals, measuring optimal clinical outcomes. The health coach conducts quarterly visits with P4YH clients to assess goal attainment, readjusting the wellness vision accordingly.
Qualifications:
Bachelor’s degree in public health, exercise physiology, nursing, dietary, counseling, or other allied health related field, or appropriate allied health associate degree and clinical certification / licensure is preferred. Certification from an accredited organization or the credentials to become certified with NBHWC (National Board of Health and Wellness Coaching) for health coaching is preferred. Other specialized certifications or training in fitness, nutrition, behavioral health, weight management, tobacco cessation, health education encouraged. Minimum two (2) years of experience in a clinical setting or in an environment working with chronic diseases is recommended.
