The Opportunity
We are seeking a highly organized and empathetic Intake Coordinator / Case Manager with experience in either law enforcement, alcohol and/or drug using clients, mental health clients or probation/parole experience - to join our team in Concord, NH. This crucial role serves as the first point of contact for clients entering our DWI program. While most client appointments are conducted via telehealth, this position will work on-site in our Concord office, managing the intake, screening, and case management process.
Key Responsibilities
- Client Screening & Intake: Conduct thorough telehealth-based screenings, gathering detailed information on clients' DWI circumstances, substance use, legal history, treatment background, and lifestyle factors.
- Assessment & Referral: Administer and score required testing tools and use results to determine if a client requires further evaluation by a licensed alcohol and drug counselor.
- Program Guidance: Clearly explain the program's rules, laws, and processes to clients to ensure full understanding and compliance.
- Case Management: Provide ongoing case management support to program participants, ensuring timely documentation and progression through the program.
- Record Keeping: Maintain meticulous, accurate, and confidential client records, utilizing computer programs (Word, Excel) daily for documentation and administrative tasks.
- Team Collaboration: Demonstrate a positive attitude and effective team-working skills as an engaged member of the Chrysalis staff. Must be a good communicator of needs and struggles, as we often other staff and leadership working remote.
Qualifications & Requirements
Required:
- Demonstrated experience in a public-facing role such as customer service, treatment or recovery, legal, probation/parole, or experience doing client intakes.
- Proven experience with professional phone-based client contact.
- High level of proficiency with computer operations, including daily use of Microsoft Word and Excel.
- Exceptional attention to detail and strong organizational skills for accurate record-keeping.
- Excellent written and verbal communication skills.
- A positive, professional, and team-oriented attitude.
Highly Desired:
- A degree (Associate's or Bachelor's) in Criminal Justice, Psychology, Social Work, or a related field.
- Previous experience in a healthcare, behavioral health, or legal services setting.

Pay is based upon qualifications and experience in the field.

Monday-Thursday from 11 am-4pm
Friday 9am-2 pm
