About the Role:
As an MDS LVN, you will work closely with the interdisciplinary team to develop, implement, and evaluate individualized care plans for our residents. Your expertise will ensure that our residents receive the highest level of care, while also promoting a positive and supportive environment.
Responsibilities:
Conduct thorough assessments and screenings to identify residents' needs and develop effective care plans
Collaborate with the care team to implement and monitor care plans, making adjustments as needed
Maintain accurate and detailed records of resident assessments, care plans, and progress
Develop and lead training programs for care staff on MDS and care planning best practices
Participate in quality improvement initiatives and contribute to the development of policies and procedures
