Behavioral Health Case Manager II (Bilingual Spanish)

We are seeking a Behavioral Health Case Manager II for a temporary assignment based in Los Angeles, CA through 12/30/2026. This role is ideal for candidates with behavioral health, healthcare, or social services experience who are passionate about helping patients navigate care and access community resources.

- Temporary Assignment through 12/30/2026 (Possible to go permanent)
- Location: 2035 Camfield Ave., Los Angeles, CA 90040
- Schedule: Monday–Friday | 8:00 AM – 5:00 PM
- Hybrid Schedule: Training onsite, then 4 days onsite / 1 day remote after training completion
- Pay Range: $24–$26/hr
- Potential opportunity for permanent placement based on openings and performance
Qualifications
- An Associate’s degree in Social Services, Psychology, Human Services, Counseling, or a related field is required (Bachelor’s degree preferred)
- Minimum 2 years of recent experience in healthcare, behavioral health, or social services required
- Experience with case management, referral coordination, and linkage to care required
- Spanish bilingual is required
Key Responsibilities
- Coordinate referrals and case management support for behavioral health and substance use patients
- Assist patients with navigating healthcare systems and accessing community resources
- Coordinate appointments and follow-up care with behavioral health providers
- Document patient interactions, referrals, and outcomes within the EHR system
- Support patients with linkage to care and higher levels of service as needed
- Answer department calls and provide assistance to patients and providers
- Collaborate with internal teams, providers, and community agencies to support continuity of care
- Participate in community events and outreach activities as needed
Ideal candidates will have strong communication skills, experience working in fast-paced environments, and a passion for supporting underserved patient populations.
Pay: $24.00 - $26.00 per hour

- High school or equivalent (Required)
