We're looking for a compassionate, energetic Personal Wellness Support Lead (PWSL) to join our Futures ALIVE Program in Concord to help participants build confidence, develop life skills, and connect with their communities.
As a Personal Wellness Support Lead, you provide support to adults with developmental and intellectual disabilities both onsite and in the community. Under the direction of the Program Coordinator and Program Supervisor, the Personal Well Support Lead is responsible for providing direct instructional services to individuals served by the agency ensuring their safety, care and welfare and promoting increased independence through onsite and community activities.
The Personal Wellness Support Lead helps facilitate community outings both local and long distance. The Personal Wellness Support Lead works closely with participants on developing and achieving meaningful goals as outlined in their Individual Service Plan. The Personal Wellness Support Lead helps manage the workflow of the program including training of Personal Wellness Support Professionals. The ideal candidate is comfortable with assisting participants with their personal care needs, community engagement, money management, and activity development.

Essential Functions and Duties:
This is a leadership role. Supplemental training will be provided however, you will be expected to:
- Spearhead the program put in your care, to provide support, training, and instructions not only to your team members (Personal Wellness Support Professionals) but more importantly, to be the front-runner for the participants’ self-advocacy, modeling appropriate behavior, effective communication, and collaborative work with their families and the communities.
- Work with program participants to create meaningful ISP goals and using Person Centered Thinking practices.
- To develop interesting programs for the participants’ self-discovery such as, art and music, outdoor experiences, reading, recreation, relaxation, traveling, vocational training and other designated skills.
- Have the appropriate training in medication administration to ensure proper medication procedures are followed.
- Willing to receive Staff-In-Charge training, including Workers’ Compensation Procedures, Sexual Harassment Prevention, Personnel Policies, Procedures, & Information, and performs duties as required of Staff-in-Charge, including health & safety officer duties, special incident reporting, and instructing and training Level I/II direct service staff in providing participant support.
- To possess applicable technological abilities, organizational as well as written skills to oversee and maintain participant database (Case Magic), permanent files, and records; making sure that all information is entered as needed/required such as, daily attendance, semi-monthly and end of month report to assigned supervisor for review and approval.
- To maintain an orderly documentation, filing and procedures according to agency, accreditation, and all regulatory statutes and standards.

Job Requirements:
- At least 18 years of age.
- Authorized to work in U.S.
- High school diploma
- 2 years or more of related work experience in a leadership position, preferably with a human service agency or educational organization collaborating with persons with intellectual/developmental disabilities
- Possess a valid CA driver’s license, insurable driving record, and evidence of auto insurance. Must be willing to drive an agency vehicle.
- Live Scan criminal record clearance (or exemption) prior to start.
- Health Screening and TB Test clearance prior to start.
- Physical stamina and ability to meet the physical requirements necessary to perform the assigned duties safely and effectively.
Great if you have:
- Knowledgeable of Lanterman Act regulations, Regional Center guidelines, and State of California Licensing regulations.
- Certified in First Aid/CPR/AED by a qualified instructor.
