Direct Support Professional (DSP) – CRA Residential
Position Summary
The Direct Support Professional (DSP) provides direct care, supervision, and person-
centered support to individuals receiving residential services through BrightStar Homes
Services. The DSP assists individuals in achieving maximum independence while
ensuring their health, safety, dignity, and overall well-being.
Services shall be provided in accordance with DBHDD Provider Standards,
NOW/COMP Waiver requirements, Individual Service Plans (ISPs), Behavior Support
Plans (BSP/CSP), Health Care Plans (HCPs), and BrightStar policies and procedures.
Essential Duties and Responsibilities
Direct Care & Individual Support
Provide direct supervision and support to individuals residing in CRA homes.
Maintain required staff-to-individual ratio (1:3), ( 1:1), (2:1) at all times.
Assist individuals with Activities of Daily Living (ADLs), including:
o Personal hygiene
o Bathing and grooming
o Dressing
o Toileting
o Meal preparation and feeding assistance as needed
Promote independence, self-advocacy, and community inclusion.
Support recreational, educational, vocational, and community integration
activities.
Implement services in accordance with:
o Individual Service Plans (ISPs)
o Behavior Support Plans (BSP/CSP)
o Crisis Safety Plans (CSPs)
o Health Care Plans (HCPs)
Utilize positive behavior supports and approved de-escalation techniques.
Avoid power struggles, intimidation, or confrontational interactions.
Follow all behavioral intervention protocols as written.
Health & Safety Responsibilities
Ensure the health, safety, and welfare of all individuals served.
Monitor individuals for changes in physical, emotional, or behavioral status.
Immediately report:
o Injuries
o Illnesses
o Behavioral crises
o Medication concerns
o Abuse, Neglect, or Exploitation allegations
Maintain line-of-sight supervision when required.
Follow all emergency response procedures.
Assist with medication administration in accordance with Proxy Caregiver training
and RN/LPN oversight.
Follow all medication administration procedures and documentation
requirements.
Report immediately all:
o Medication errors
o Medication refusals
o Medication changes
o Adverse reactions
Notify the CRA Home Supervisor, Medical Compliance Staff, Case Management,
and Leadership immediately of:
o ER visits
o Urgent Care visits
o Hospitalizations
o Police contacts
Provide discharge paperwork and medical summaries to the CRA Home
Supervisor and Medical Compliance Staff immediately upon return.
Meal Preparation & Household Duties
Prepare and serve all meals and snacks in accordance with prescribed diets and
physician orders.
Maintain a clean and sanitary kitchen and dining area.
Assist individuals with household chores and daily living skills.
Maintain inventory of food, household supplies, and personal care items.
Report supply shortages to the CRA Home Supervisor.
Complete all required documentation accurately and timely in the Therap system.
Document:
o Daily service notes
o Progress toward ISP goals
o Medication administration records
o Health tracking forms
o Behavioral observations
o Community activities
Ensure documentation is objective, factual, and complete.
Report documentation concerns to the CRA Home Supervisor immediately.
Incident Reporting
Immediately report all incidents to the CRA Home Supervisor and/or Leadership.
Cooperate fully with internal investigations.
Complete required incident documentation before the end of shift.
Maintain confidentiality regarding all investigations.
Never discourage or interfere with an individual's right to report concerns.
Environmental Safety
Maintain a safe, clean, and organized home environment.
Complete assigned housekeeping duties.
Assist with fire drills and emergency preparedness activities.
Report maintenance concerns and safety hazards immediately.
Pay: From $16.00 per hour
