Duties
- Conduct thorough patient assessments including vital signs, physical examinations, and medical history documentation.
- Administer medications and injections following proper aseptic techniques and medication protocols.
- Monitor patients’ physiological status using advanced equipment such as ventilators, IV infusion devices, and catheters.
- Manage complex care needs including wound suturing, ostomy care, tube feeding, and infection control measures.
- Collaborate with interdisciplinary teams to develop and implement individualized discharge plans aligned with patient needs and preferences.
- Educate patients and families on disease management, medication administration, nutrition, and post-discharge care instructions.
- Document all patient interactions accurately within electronic health records (EHR systems) like Epic or Cerner while adhering to HIPAA regulations.
- Participate in quality improvement initiatives aligned with NCQA standards and other regulatory requirements.
- Provide specialized care for diverse populations including pediatrics, geriatrics, behavioral health patients, or those with complex chronic conditions such as dementia or Alzheimer’s disease.
- Assist in diagnostic evaluations involving laboratory specimen collection/processing and medical imaging procedures such as sonography or mammography when required.
- Maintain sterile processing protocols for equipment sterilization and ensure infection control standards are consistently met.
Pay: $35.00 - $60.00 per hour
Experience:
- Skilled Nursing: 1 year (Preferred)
