Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition
- Support medication administration following prescribed instructions and medical documentation protocols
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments
- Help with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment
- Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Assist with transfers using equipment like Hoyer lifts or other mobility aids while ensuring safety standards are upheld
- Support end-of-life and hospice care needs with compassion and respect for patient dignity
- Follow home healthcare regulations and maintain compliance with HIPAA privacy standards
- Collaborate effectively with nursing staff and other healthcare providers to deliver comprehensive patient care
Requirements
- Proven experience working in assisted living facilities, nursing homes, or home healthcare environments
- Knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring
- Experience caring for geriatric patients or individuals with developmental disabilities or dementia
- Ability to perform heavy lifting and physical tasks safely while maintaining patient safety standards
- Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS) required
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Strong understanding of home & community care regulations and HIPAA compliance requirements
- Excellent caregiving skills combined with patience, empathy, and strong communication abilities
- Valid driver’s license preferred for travel between patient locations; live-in employment experience is a plus
