Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, noting vital signs and behavioral changes to inform care plans
- Administer medications accurately following prescribed instructions while maintaining strict compliance with HIPAA regulations
- Prepare nutritious meals aligned with dietary restrictions and personal preferences to promote health and well-being
- Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to ensure safe transfers and mobility assistance
- Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces
- Document care activities meticulously, including patient observations, medication administration, and progress notes in compliance with healthcare standards
Experience
- Prior experience in home health, assisted living, nursing homes, or group homes is highly valued
- Demonstrated knowledge of patient care techniques including Alzheimer’s care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or autism
- Familiarity with vital signs monitoring, medication management, and emergency response procedures such as CPR and first aid certification
- Experience working with residents who have disabilities or require long-term care support in community or home settings
- Ability to perform heavy lifting safely when necessary for resident transfers or mobility assistance
- Strong understanding of HIPAA regulations to ensure confidentiality and privacy of patient information
- Compassionate attitude with excellent communication skills to collaborate effectively with clients’ families and healthcare teams
