Location: Meeker, CO 81641(Must meet the facility's 65-mile radius rule; local travelers will not be considered.)

Job Description
Pioneers Medical Center is seeking an experienced Registered Nurse (RN) to provide patient care in a Long-Term Care (LTC) setting. The RN will be responsible for providing safe and compassionate nursing care, monitoring patients, administering treatments and medications, and working with the healthcare team to meet patient needs.
Responsibilities
- Provide direct nursing care to patients in a Long-Term Care setting.
- Assess patients and monitor changes in their condition.
- Administer medications and treatments according to physician orders and facility policies.
- Maintain accurate and timely nursing documentation.
- Communicate patient changes and concerns to physicians and other members of the care team.
- Develop and follow individualized patient care plans.
- Coordinate care with CNAs, other nurses, providers, and support staff.
- Provide patient and family education as needed.
- Maintain patient safety and follow infection-control and facility procedures.
- Respond appropriately to changes in patient condition and emergencies.
- Maintain a patient assignment ratio of approximately 1:5.
- Participate in required on-call and holiday coverage.
- Provide care in accordance with Colorado regulations and facility standards.
Required Qualifications
- Current, unrestricted Colorado RN license – Required
- BLS/BCLS – Required
- Minimum 4 years of RN experience
- Long-Term Care patient experience – Required
- Current and active nursing credentials
- Ability to work day shifts
- Must be willing to work holidays
- Must meet the facility's 65-mile radius rule; local travelers will not be considered.
- COVID vaccination required per applicable state mandate
- Ability to work effectively in a patient-centered healthcare environment
Certifications
- BLS/BCLS – Required
- ACLS
- PALS
- TNCC
Pay: $60.00 - $65.00 per hour
