Patient and Oncology Care Coordinator
Job Description
We are seeking an Oncology Care Coordinator with a strong attention to detail and strong organizational and communication skills to join our team. Our practice strives to provide clients and patients with the best possible oncology experience during a very difficult time. The coordinator will ensure that every patient and client moves through the system smoothly allowing for the best care for the patient and best experience for the client.
What you’ll do:
- Ensure smooth patient workflow by coordinating with oncologist, technicians and client hospitality specialists (our CSRs)
- Prepare cases before the oncologist sees the patient
- Ensure records, estimates, and medications are ready before appointments
- Ensure we have all necessary patient records, imaging, labs and labs prior to patient visits
- Ensure all charges are accurate and finalized prior to discharge
- Ensure clients leave with everything needed (refills, discharge instructions, recheck booked, invoice emailed or printed)
- Responsible for tracking laboratory and imaging results, prescriptions, and treatment plans so nothing is missed
- Track patients through care journey (ex. follow up after surgery or radiation, etc.) so they receive appropriate care
- Act as the primary liaison for referring veterinarians (including sending updates, imaging, labs, records)
- Assist clients in scheduling referrals with other specialists (including providing contact information, assessing availability)
- Ensure recommended follow-up occurs and resolve delays before they affect treatment
- Assist team in responding to emails and texts
- Proactively solve problems (create client handouts, improve workflows)
- Learn chemotherapy side effects and treatment protocols for common cancers
- Perform additional duties as assigned to support clinic operations and excellent patient care
Ideal candidate:
- Experience in veterinary specialty medicine (including RVT) or human surgery, dermatology, concierge medicine, or another high-touch client-facing environment
- Excellent phone, text, and email communication skills with the ability to explain medical information clearly and empathetically
- Ability to deliver exceptional client service in fast-paced clinical environment
- Experience in care coordination, case management or patient navigation preferred
- Professional with the ability to navigate sensitive conversations thoughtfully and respectfully
- Highly organized and detail-oriented
- Self-motivated and thinking ‘how can this be improved?
- Yes-minded (find a way to help a client, patient or team member; thinking outside of the box is encouraged)
- Reliable, proactive, and desire to take ownership
- Proficiency in electronic medical records and related systems, including Google Workspace
- Ability to interpret laboratory reports, clinical documentation, and data with accuracy and attention to detail
- Willingness to work four 10-hour shifts (currently Monday-Thursday)
