Provide skilled, compassionate nursing care including symptom management, emotional support, patient/family education, and timely response to needs
Deliver care through routine home visits, end-of-life/continuous care, referrals, admissions, care center support, and chart reviews
Perform thorough initial nursing assessments and monitor patient condition
Explain hospice services, insurance benefits (Medicare, Medicaid, private), palliative care interventions, and appropriate use of ABNs
Collaborate with physicians and Medical Directors and obtain/enter orders as appropriate
Order and coordinate Durable Medical Equipment (DME) to meet patient needs
Facilitate continuity of care, including home-to-inpatient transitions (hospital, nursing home)
Complete accurate, timely documentation in compliance with agency, accreditation, and regulatory standards
Maintain clinical competency and participate in weekend rotations or other duties as assigned
