Job Summary

Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews to inform care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
- Perform specialized procedures such as suturing, catheterization, tube feeding, and sterile processing to support patient recovery and comfort.
- Utilize electronic health record (EHR) systems like Epic, Cerner, Athenahealth, or eClinicalWorks for precise documentation of patient care activities.
- Coordinate with multidisciplinary teams to develop discharge plans, manage case documentation, and ensure seamless transitions of care across settings such as hospital, outpatient clinics, or home care.
- Provide triage services by assessing urgent patient needs and prioritizing interventions in emergency room or urgent care environments.
- Support infection control protocols and aseptic techniques to maintain a safe environment for patients and staff.
Skills
- Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal, behavioral health, and individuals with developmental disabilities.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and laboratory procedures.
- Proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for efficient medical documentation and record management.
- Skilled in critical care practices including ICU experience, trauma medicine (Level I & II Trauma Center experience), cardiac catheterization, ventilator management, and acute pain management.
- Expertise in specialized areas such as hospice & palliative medicine, infectious disease care, dialysis procedures, ostomy care, neurovascular assessments (CT scans), and long-term care environments like nursing homes or assisted living facilities.
- Ability to perform complex procedures such as venipuncture (phlebotomy), IV insertion/infusion therapy, tube feeding management, and catheter care with precision.
- Excellent communication skills for patient education on health coaching topics including medication administration and discharge planning while adhering to HIPAA regulations.
