Job Summary:
We are seeking a compassionate and dependable Registered Nurse (RN) to join our Home Health Agency. This job is Per Diem and we are very flexible: you choose your schedule and your preferred areas.
Each visit is between 20 min to 1 hour long. Depending on your availability, you will receive 1-10+ visits per week, for each patient assigned to you.
Key Responsibilities:
- Perform comprehensive nursing assessments for patients in their homes.
- Develop and implement individualized care plans in collaboration with patients, families, and physicians.
- Provide skilled nursing services such as wound care, medication administration, IV therapy, disease management, and patient education.
- Monitor patient conditions and report changes to the supervising physician and care team.
- Document all patient care accurately and timely in compliance with regulatory and agency standards.
- Coordinate with interdisciplinary team members including physical therapists, occupational therapists, social workers, and home health aides.
- Educate patients and families on managing conditions, medication adherence, and healthy lifestyle choices.
- Maintain patient confidentiality and comply with HIPAA regulations.
Qualifications:
- Current RN license in Nevada with no restrictions
- At least one year of clinical nursing experience, preferably in home health or a similar setting
- BLS (Basic Life Support) Certification required
- Valid driver's license and reliable transportation
- Familiarity with electronic medical records (EMR) systems
- Ability to work independently and manage time efficiently
Preferred Qualifications:
- Experience with patient documentation on computer
- Knowledge of Medicaid regulations
- Spanish Speaking Bilingual is a plus
