Position Overview
The Hospice Chaplain provides spiritual and emotional support to patients, families, and caregivers, respecting all beliefs, cultures, and values.
This role is an integral part of the interdisciplinary team, ensuring comprehensive, person-centered care.
Key Responsibilities
Provide spiritual care and counseling to patients and families
Conduct spiritual assessments and develop care plans
Offer grief and bereavement support
Collaborate with interdisciplinary team members
Support diverse religious and cultural backgrounds
Participate in IDT meetings
Maintain accurate documentation
Qualifications
Ordained or endorsed by a recognized religious/spiritual organization
1+ year chaplaincy or spiritual care experience preferred
CPE units preferred
Strong interpersonal skills
Valid driver’s license and reliable transportation
