Duties:
- Provide direct and indirect patient care in a variety of healthcare settings
- Perform assessments, develop care plans, and implement nursing interventions
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and report any changes or abnormalities
- Provide education and support to patients and their families on various healthcare topics
- Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
- Document patient care activities accurately and timely
Qualifications
- Minimum of 2 years of experience as a licensed Registered Nurse
- Experience in Hospice or Home Health Care preferred
- Proficient in planning and coordination of care for patients transitioning to home or other healthcare facilities
- Ability to navigate sensitive conversations with empathy and clarity
- Knowledge of Durable Medical Equipment (DME) and its usage in patient care
Requirements:
- Valid Registered Nurse license in LA
- Current CPR certification
- Strong communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
- Attention to detail and ability to maintain accurate documentation
