About the Job
The Care Manager conducts youth-centered service meetings to determine client’s strengths and needs and then what services are needed to address those needs. Provides general client services, group activities, programming, and case management in a Montgomery County Public School.
Position: Care Manager Salary Range: $50,000 - $52,000
Some of the exciting tasks you will do as a Care Manager include but are not limited to:
- Accepts referrals for social services and completes initial intakes
- Consults with county/community/agencies to identify and develop potential resources for client assistance
- Provides regular checkins for teens as indicated
- Designs intervention strategy youth and/or youth’s family to match appropriate community resource to the youth and family’s needs and makes those connections
- Provides case management and monitoring services to youth/families
- Creates and executes student engagement activities for teens including weekly groups, workshops and activities
- Provides on-going social services (appropriate assistance during crises, may provide supportive counseling, food, clothing, housing, employment, transportation, financial assistance)
- Performs accurate recordkeeping including intakes, developing, reviewing and updating the Life works assessment, contact notes and electronic database entries
Qualifications and Skills:
- Bachelor’s degree in psychology, social work, sociology, criminal justice, family science or other human service-related field.
- 1-3 years’ experience developing and administering teen centered programming
- Experience in working with diverse populations; and skilled at collaborating with multiple agencies
- Bi-lingual in English and Spanish preferred
- Relationship building (youth, parents, community, etc.)
- Clear, effective written and verbal communication
