Summary
We are seeking a detail-oriented Designated Coordinator to join our team at Epic Care Home Services.
The DC provides supervision, support and evaluations of activities for individuals receiving services. The DC complies with Coordinated Service and Support Plans for individuals in according to federal and state regulations and the organization’s policies and procedures to ensure the quality delivery of services to all individuals served. The Designated Coordinator is required to be competent to perform the required duties through education, training, and work experience relevant to the primary disability of persons served by the license holder and the individual persons for whom the Designated Coordinator is responsible.
Requirements
- 2-3 years experience as a DC is required
- Excellent attention to detail with strong organizational skills.
- Ability to communicate effectively with clients and healthcare professionals.
- Driver's license
