Weekend Premium Shift Differentials
- Evenings and Nights (Fri–Sun): Base Rate ($30/hr) + $9.50 per hour, up to $39.50
- Days (Sat–Sun): Base Rate ($30/hr) + $7.00 per hour, up to $37.00
Responsibilities:
1. Implement and coordinate care delivery in collaboration with Nursing team
2. Conduct regular health and wellness checks for residents.
3. Assist in developing and updating individualized service plans for residents.
4. Maintain open communication with residents, families, and physicians regarding their care needs.
5. Utilize the nursing process (assessment, planning, and implementation, evaluation) to ensure optimal resident care.
6. Other duties as assigned
Requirements:
1. Current NYS LPN License in good standing.
2. BLS/CPR Certificate
3. Ability to work effectively in a team-oriented environment.
4. Prior experience in a nursing home or hospital is preferred but not required.
Pay: $37.00 - $39.50 per hour
