Transmountain Therapy is a locally-owned home health therapy contract group serving El Paso, TX and Las Cruces, NM. We've been operating for 3+ years with 30+ active therapy contractors. Founded and run by a practicing clinician — not a staffing recruiter — so the people making decisions about your caseload and pay are people who do the work.
What You'll Do
- Evaluate home health patients in their homes and establish individualized plans of care
- Treat patients on your caseload — gait training, balance, therapeutic exercise, manual therapy, neuromuscular re-education, and patient/family education
- Perform re-certifications and discharge planning
- Supervise our experienced PTA team in the field (collaborative — not babysitting)
- Attend weekly case conferences with partner home health agencies
- Document evaluations, follow-up visits, and discharges in the partner agency's EMR (WellSky/Kinnser, HCHB, MatrixCare, or Axxess)
- Choose your zones and your visit volume — we work around your availability
Pay and Earnings
- $75 to $80 per visit (paid per visit completed, including documentation)
- Real weekly earnings examples based on visit volume:
- 10 visits/week: ~$750–$800 before taxes
- 15 visits/week (part-time PRN): ~$1,125–$1,200 before taxes
- 20 visits/week (full-time PRN): ~$1,500–$1,600 before taxes
- Annual earnings potential at full-time PRN: $60,000–$80,000+
- Fast direct deposit — paid biweekly
- 1099 contractor — you handle your own taxes; rates account for that

Requirements
- Active Texas Physical Therapist (PT) license in good standing
- Valid driver's license, reliable vehicle, and current auto insurance
- Current CPR/BLS certification
- Professional liability/malpractice insurance
- Comfortable working independently in patient homes
