About the Role
The Virtual Physician partners with clinical leadership to translate strategy into deployed workflows and production-grade tools. The role includes refining care pathways and caring for a patient panel to generate real-world feedback that continuously strengthens the model.
This position offers hands-on collaboration with product teams, the opportunity to launch new clinical capabilities, see patients directly in an AI-enabled chronic care model, and redefine care delivery alongside clinical AI. In this role, you will serve as an expert-in-the-loop: the clinical voice that ensures we deliver safe, high-quality care at scale.
This is a part-time, flexible role.
Reporting to the Chief Clinical Officer (CCO), you will play a pivotal role in driving client success and guiding Altitude’s future direction.
What You’ll Do
- Chronic Condition Management Delivery:
- Function as a virtual physician delivering chronic condition management and transitions of care virtual visits for a defined patient cohort, first manually and later supported by Altitude’s AI models
- Iteratively improve care delivery across longitudinal care cycles
- Surface operational friction points and guide workflow redesign to enhance quality, efficiency, and patient experience
- Codify best practices and minimize clinical variation
- Serve as a clinical resource across the company, and particularly to clinical team members.
- Product Refinement:
- Institutionalize feedback loops through regular case review and quality audits
- Provide critical feedback on care model and product with clinical, product, and engineering teams
- Evolve clinical scorecards and carry out a closed-loop safety event process from reporting through resolution in conjunction with the clinical product team
What We’re Looking For
- Required qualifications
- MD with active or eligible U.S.-based licensure
- Board certification in Family Medicine or Internal Medicine, interest and focused experience in heart failure or diabetes management a plus
- Minimum 2 years of post-residency care delivery - primary care highly preferred
- Active or recent clinical practice (within past 3 years)
- Experience with managing complex chronic care patients
- Demonstrated ability to provide insight on clinical processes and implement them operationally
- Energized by early-stage environments, and experience operating in high-growth clinical environments such as telehealth, value-based care, or scaling practices
- Tech forward, experience with clinical AI products
- Loves working with patients: high empathy and good interpersonal skills
