Position Summary
The Case Manager / Success Partner supports the delivery of Mane Pathways™, Mane Steps to Money™, Lunch and Learns, Enrollment Sessions, and other Mane Up Memphis™ prevention-focused behavioral health and emotional regulation programs serving youth, young adults, and families.
This role focuses on participant engagement, accountability, case coordination, documentation support, emotional wellness reinforcement, and continuity of care throughout the program cycle. The Success Partner works closely with Program Therapists, Enrollment Coordinators, and internal staff to ensure participants remain connected, engaged, and supported throughout services.
This position is designed to support prevention, emotional regulation, relationship skills, behavioral wellness, and community stabilization through structured engagement, accountability support, and resource coordination.
Work Schedule
Saturdays 8am-8pm as needed/schedule
Tuesdays and/or Thursdays 8am -5pm as needed/schedule
Core Responsibilities
Program Delivery & Participant Support
Support Mane Pathways™ group sessions in alignment with curriculum standards and program structure
Assist with participant engagement, attendance tracking, accountability check-ins, and group support during sessions
Reinforce emotional regulation strategies, communication skills, and wellness education introduced by Program Therapists
Support in-person and telehealth programming as assigned
Assist with makeup sessions, workshops, outreach activities, and community events connected to Mane Up Memphis™ programming
Case Management & Care Coordination
Conduct weekly participant check-ins to support engagement, accountability, and goal progression
Monitor participant attendance, participation, and program completion progress
Collaborate with Program Therapists and Enrollment Coordinators to ensure participants are properly enrolled in cohorts, makeup sessions, and continued services when applicable
Assist participants with referrals and connection to community resources and support services
Escalate participant concerns, behavioral concerns, or crisis-related issues to licensed clinical staff when appropriate and/or assigned to by Program Operations Manager
Support continuity of care and communication between participants, families, schools, and internal staff when applicable
Documentation & Compliance
Complete attendance tracking, engagement and progress notes, follow-up documentation, and required program documentation
Maintain HIPAA compliance and confidentiality standards
Utilize Apricot and approved systems for assigned documentation responsibilities
Maintain accurate and timely updates within Apricot and internal reporting systems to support quality assurance and real-time program tracking
Participate in required trainings, supervision meetings, program rehearsals, and quality improvement activities as assigned
Collaboration
Work closely with Program Therapists, Enrollment Coordinators, and the Program Operations Manager to maintain smooth program delivery
Coordinate with Case & Data staff regarding participant engagement and referrals
Communicate professionally with staff, schools, community partners, and families when applicable
Support outreach events, workshops, and community engagement activities as assigned
Qualifications
Bachelor’s degree or master’s degree in social work, psychology, counseling, human services, public health, or a related field preferred
LMSW candidates are encouraged to apply
This position does not provide supervision hours toward independent clinical licensure
Strong communication, organization, and relationship-building skills
Experience working with youth, young adults, and families preferred
Ability to work collaboratively in fast-paced community environments
Comfortable with both in-person and telehealth engagement

Schedule Expectations
Flexible scheduling based on cohort and program needs
- Must be available annually for the first Saturday in May for the Mane Mindscape™ Mental Health Festival
Evening and Saturday availability preferred
Typical program coverage may include Monday, Wednesday, Friday and sometimes Saturday
Compensation Structure
Paid per approved session, workshop, or assigned program activity
No guaranteed minimum hours
Compensation determined by approved contractor agreement and program assignment needs
