Job Summary
We are seeking a dedicated and compassionate Registered Nurse for our Home Health division. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while collaborating with a multidisciplinary team.
Duties
- Conduct comprehensive assessments of patients' health status in their home environment.
- Develop and implement individualized care plans based on patient needs and preferences.
- Administer medications and treatments as prescribed, ensuring adherence to aseptic techniques.
- Monitor vital signs and manage ventilatory support for patients requiring respiratory care.
- Provide triage services for patients experiencing acute health issues or complications.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
- Educate patients and families on health management, disease prevention, and home care techniques.
- Document all patient interactions accurately in compliance with regulatory standards.
Skills
- Proficiency in laboratory procedures and understanding of clinical diagnostics.
- Experience in Pediatric Intensive Care Unit (PICU) or trauma medicine is preferred.
- Strong knowledge of home community care practices and medical office operations.
- Ability to perform triage effectively in various healthcare settings.
- Excellent interpersonal skills to build rapport with patients and families.
- Strong organizational skills to manage multiple patient cases efficiently.
- Commitment to providing compassionate care tailored to individual patient needs.
