Responsibilities
- Conduct thorough patient assessments to evaluate nutritional needs, medical history, and lifestyle factors using electronic health records (EHR) systems.
- Develop and implement individualized care plans tailored to chronic disease management, weight loss, geriatrics, and acute care needs.
- Provide expert guidance on nutrition therapy including tube feeding management, dietary modifications, and diagnostic evaluation of nutritional deficiencies.
- Utilize motivational interviewing techniques to motivate behavioral change and support long-term health improvements.
- Oversee and supervise nutrition services delivered by staff or interns, ensuring adherence to clinical standards and best practices.
- Collaborate with healthcare teams to coordinate patient care plans, including hospital discharge planning and long-term care strategies.
- Maintain accurate documentation of patient interactions, progress notes, and treatment outcomes in EMR (Electronic Medical Records) systems.
- Stay current with the latest research in clinical nutrition, physiology knowledge, and dietary guidelines to inform practice.
- Educate patients on dietary strategies for weight loss, chronic disease management such as diabetes or cardiovascular conditions, and geriatrics-specific nutrition needs.
- Support the dietary department by ensuring compliance with insurance requirements and quality standards for patient services.
Experience
- Registered Dietitian (RD) certification from an accredited program; Certified Dietitian Nutritionist (CDN) credentials are preferred.
- Proven experience in clinical nutrition within hospital or long-term care environments.
- Demonstrated supervising experience with dietetic interns or staff members in a healthcare setting.
- Extensive knowledge of physiology related to digestion, metabolism, and disease processes affecting nutrition.
- Hands-on experience managing tube feeding protocols and care plans for complex patients.
- Familiarity with EMR/EHR systems such as Epic or Cerner for documentation and patient record management.
- Background in chronic disease management programs focusing on weight loss, geriatrics, or acute care populations.
- Strong skills in nutritional analysis, diagnostic evaluation of nutritional status, and developing comprehensive care plans.
- Experience working within insurance-based practices ensuring compliance with billing standards and documentation requirements.
