Licensed Practical Nurse (LPN) – LTC

Assignment Duration: 13 Weeks
Schedule: 8-hour Evening/night shift (5 shifts per week)
Hours: 37.5-40 hours/week (Guaranteed)

Pay Package
- Hourly Pay: $36/hr-$40/hr
Job Overview
- Provides compassionate nursing care to long-term care residents, including medication administration, treatments, assessments, and ongoing monitoring of residents’ health conditions.
- Coordinates daily resident care with physicians, RNs, CNAs, families, and other healthcare professionals while maintaining accurate clinical documentation and ensuring compliance with established care plans and facility standards.
Key Responsibilities
- Provide direct nursing care to long-term care residents, including medication administration, treatments, vital signs, assessments, and ongoing monitoring.
- Implement and follow individualized care plans, coordinating with RNs, physicians, CNAs, residents, and families to ensure continuity of care.
- Maintain accurate and timely clinical documentation, monitor changes in residents’ conditions, and communicate concerns to the appropriate healthcare providers.
Job Requirements
- Active NY LPN license required.
- BLS (AHA) certificate required
- Recent long-term care/skilled nursing facility (LTC/SNF) experience required.
Contract & Pay Details
- Contract Type: Local/Travel both candidates are accepted
- Guaranteed Hours: 37.5-40 hours/week
- On-Call: Not required
