JOB SCOPE:
This is a direct service position requiring specific knowledge of mental health, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires the desire to work as part of a team.
ESSENTIAL FUNCTIONS:

Participate in morning meetings and staff meetings.

Evaluate need for psychotropic medication; prescribe medications as needed, educates clients and relatives about these medications and their effects, reviews and adjusts dose level as needed, and documents all medications in consumer’s electronic chart.

Complete pyschiatric evaluations and updates
- Conduct visits in the community in accordance with NYS Office of Mental Health (OMH) licensing standards.

Coordinate consumer access to medications with the RN and LPN nurses including administering injectable medications when needed.

Order routine blood work and/or toxicology testing as needed.
EDUCATION AND EXPERIENCE:
- Master's degree in advanced nursing from an accredited Nurse Practitioner Program; Current license in NYS as a Psychiatric Nurse Practitioner; NYS license as a Registered Nurse.

Enrolled in NYS Medicaid required. Enrolled in PECOS, preferred.
- Minimum of three (3) years working with people with serious mental health conditions, co-occurring substance use disorders, and homelessness.
