- High School diploma or GED.
- 1 year experience directly related to the duties and responsibilities of clinical health and human services.
- Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
- Must have American Heart Association Healthcare Provider CPR and First Aid or able to obtain within 90 days after hire.
- Knowledge of community agencies and resources.
- Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
- Demonstrated proficiency in grants management, public speaking, group facilitation, training, prevention program development, prevention evaluation, strategic planning, and implementation, and data compilation.
- Understand principles, practices and techniques of grant project planning, development, evaluation, procedures of budget preparation and monitoring, and contracted services.
Knowledge, Skills and Abilities
- Must be reliable, detailed oriented, capable of multi-tasking with excellent written and verbal communication skills as well as strong professional customer- service interpersonal skills.
- Ability to adjust to changing conditions.
- Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
- Ability to plan, implements, and evaluate individual client care plans.
- Ability to communicate medical information to health care professionals and care coordinators over the telephone.
- Must be computer literate required (Windows operating systems – Microsoft Word, Excel).
- Must have a valid Oklahoma Driver’s License.
- Must be able to pass a background check and drug test.
Physical Requirements and Working Conditions
- Must be able to sit, stand, stoop, bend or kneel for long periods of time.
- Prolonged sitting, standing, or walking; occasional bending, squatting, kneeling, and stooping; good finger dexterity and sensory perception; frequent repetitive motions; as well as talking, hearing, and visual acuity are required.
