Job Summary:
OverSightMD is seeking a compassionate and skilled Clinical Social Worker to join our Enhanced Care Management team in Merced, California. The Clinical Social Worker will provide intensive case management, psychosocial support, and care coordination advice to our lead care manager teams and direct support to clients with diverse needs, including chronic health conditions, mental health challenges, and social determinants of health. This role requires a proactive, empathetic professional who thrives in a dynamic, community-focused setting.
Key Responsibilities:
- Conduct comprehensive assessments to identify client needs and develop individualized care plans.
- Provide guidance to lead care management teams conducting intensive case management for members enrolled in ECM services.
- Coordinate with healthcare providers, community resources, and multidisciplinary teams to ensure seamless care delivery.
- Facilitate access to social services, such as housing, transportation, and financial assistance, to address social determinants of health.
- Deliver evidence-based therapeutic interventions, including motivational interviewing and cognitive-behavioral techniques.
- Maintain accurate and timely documentation in compliance with Medi-Cal, ECM, and OverSightMD standards.
- Participate in team meetings, case conferences, and ongoing professional development to enhance service quality.
- Advocate for members’ rights and empower them to achieve their health and wellness goals.
Qualifications:
- Required:
- Master’s degree in Social Work (MSW) from an accredited institution.
- Active Licensed Clinical Social Worker (LCSW) credential in California.
- Minimum of 1 year of clinical social work experience, preferably in case management or community health settings.
- Knowledge of Medi-Cal, Enhanced Care Management, and Santa Clara County social service systems.
- Strong interpersonal, organizational, and crisis intervention skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
- Valid California driver’s license and reliable transportation (travel within Santa Clara County required).
- Preferred:
- Bilingual proficiency (Spanish, strongly preferred).
- Experience working with populations experiencing homelessness, substance use disorders, or chronic health conditions.
- Familiarity with trauma-informed care and harm reduction principles.
