At Home Health Care, Inc., we're looking for a Physical Therapist or Occupational Therapist who wants to make an impact far beyond the patients they personally see.
Instead of carrying your own full caseload, you'll become a mentor, coach, and trusted resource for clinicians serving more than 1,700 home health clients across our 21-county Twin Cities service area.
You'll help therapists, nurses, and home health aides navigate complex situations, improve outcomes, and deliver exceptional care where patients want to be most—home.

Become the Person Clinicians CallClinical Leadership & Mentorship
- Coach and mentor therapists, nurses, and home health aides
- Serve as a clinical resource for field staff and complex patient situations
- Conduct case reviews, audits, and field visits
- Support clinicians through transitions of care and challenging cases
- Promote a culture of collaboration and clinical excellence
Quality & Operational Excellence
- Ensure evidence-based care and regulatory compliance
- Lead quality initiatives focused on outcomes and patient satisfaction
- Monitor key metrics including hospitalizations and quality measures
- Collaborate with intake, scheduling, and operations teams
- Participate in strategic growth and service expansion initiatives

QualificationsRequired
- Active Minnesota PT or OT license
- Previous Home Health experience
- Strong understanding of Medicare regulations, OASIS, and home health compliance
- Strong clinical decision-making and communication skills
- Ability to thrive in a fast-paced, collaborative environment
Preferred
- Leadership, supervisory, preceptor, or mentoring experience
- Experience with EMR systems and electronic clinical documentation
- Passion for developing and supporting clinicians
