Part-Time Registered Nurse (RN) – HCS Experience Preferred
New Journey Home & Health Care Services is seeking a compassionate, dependable, and motivated Registered Nurse (RN) to join our growing team on a part-time basis. We are looking for someone who is experienced in Home and Community Services (HCS) and passionate about providing quality care and support to individuals in the community.
Position Summary
The RN will assist with nursing assessments, monitoring client health needs, ensuring compliance with HCS standards, supporting direct care staff, and working collaboratively with the interdisciplinary team to provide quality client care.
Responsibilities
- Conduct nursing assessments and health evaluations
- Develop and review individualized care plans
- Monitor client health status and nursing needs
- Train and support direct care staff as needed
- Maintain accurate nursing documentation and compliance records
- Communicate effectively with families, providers, pharmacies, and team members
- Participate in weekly staff meetings
- Work collaboratively as part of a team-oriented environment
- Assist with maintaining HCS compliance and quality standards
Qualifications
- Active Texas Registered Nurse (RN) license
- HCS, home health, or community-based services experience preferred
- Strong communication and organizational skills
- Dependable and professional
- Must be a team player with a positive attitude
- Reliable transportation required
