About the Role
As an Intake Coordinator at CareM Support, you are the front door to care — the first connection in a patient’s journey. In this role, you’ll coordinate referrals and admissions with precision and urgency, helping ensure patients are onboarded smoothly into home-based services.
This is a fast-paced, high-impact position where your ability to communicate clearly, stay organized, and manage competing priorities directly supports patient care. You’ll work alongside a strong, collaborative team that relies on each other to get important work done well — and done right.

Key Responsibilities

Receive and process patient referrals via phone, fax, email, and in-person communication

Verify insurance benefits and communicate eligibility clearly to internal teams and referral sources

Enter and maintain accurate patient data in EMR systems

Coordinate start-of-care details, including patient outreach and physician verification

Track documentation and follow up with internal teams to ensure timely completion

Collaborate with clinical, billing, and operations teams to support seamless patient onboarding
Requirements:
What We’re Looking For

1–2+ years of experience in healthcare intake, admissions, scheduling, or a related role (preferred)

Strong communication skills — clear, professional, and responsive

High attention to detail, especially with data entry and documentation accuracy

Ability to manage multiple priorities in a fast-paced environment without losing focus

Basic understanding of insurance verification and healthcare workflows

A calm, steady presence — someone who can stay organized and composed under pressure
