As our RPM Coordinator, you will be the heart of our remote care program. This role is a perfect blend of clinical data oversight and patient connection. You will work independently to monitor incoming vitals, conduct monthly educational calls, and ensure our program meets strict monthly time and compliance tracking requirements. Because we are a small team, you will also be cross-trained in other clinic areas, ensuring a dynamic work environment where you are never just a cog in a machine.
Key Responsibilities:
- Vitals Monitoring & Alert Response: Review incoming blood pressure and weight data daily. Actively identify and respond to "out-of-range" alerts in a timely, calm, and professional manner.
- Patient Education & Connection: Conduct monthly phone conversations with enrolled patients, providing vital education on managing their numbers, checking in on their well-being, and keeping them engaged in their health journey.
- Physician & Supervisor Collaboration: Regularly communicate critical alerts or patient trends to our physicians. Provide routine status reports to your supervisor regarding call metrics and program health.
- Compliance & Time Tracking: Diligently track and log your interaction time per patient, ensuring both the clinic and the patients meet the monthly regulatory requirements for readings and monitoring minutes.
- Team Cross-Training: Participate in cross-training to support our small office environment when needed, fostering a seamless experience for our patients.
What We Are Looking For:
- The Gift of Gab (with a Medical Twist): Exceptional telephone communication skills. You must love talking to people, exhibit deep empathy, and possess the ability to explain medical monitoring concepts clearly to patients.
- Medical Background: Background as a Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Health Coach, or equivalent experience in medical monitoring/education.
- Tech-Savvy & Independent: High comfort level navigating computer software, electronic health records (EHR), and RPM data dashboards with minimal supervision.
- Detail-Oriented: A knack for tracking time, numbers, and compliance metrics accurately.
- Team Player: Someone who thrives in a small, collaborative, supportive office environment and enjoys learning new skills.
Pay: From $18.00 per hour
