Nitomek Health Care Services has an immediate opening for a Registered Nurse (RN) or License Vocational Nurse (LVN). The RN/ LVN will be under the general supervision of the RN Supervisor. She/he provides intermittent skilled nursing services, communicates the patient's progress with other disciplines and directs, supervises and instruct non-professional home health aide staff in the provision of personal care to the patient. The ideal candidate will have previous Home Health experience, as well as a flexible schedule. Must be willing to travel throughout Houston Metro and surrounding areas ( Galveston, Texas City, Cypress, Katy, Rosenberg, Richmond )
Qualifications:
A. Is currently licensed in the State of Texas in accordance with the Board of Nursing Examiners rules for Nurse Licensure Compact (NLC) and
B. One (1) year experience as home health nursing required.
C. Ability to work in the field setting and exhibited ability to make sound nursing judgement.
D. Ability to assess patient needs and formulate individualized patient care plans to meet those needs.
E. Effective communication skills
F. Must have and maintain an automobile to be used for work. (Auto insurance, Driver license)
G. Willingness to travel throughout Houston and surrounding areas.
Responsibilities:
- Provide home care to patients after illness, injury or surgery or with acute health conditions or patients who are being treated with pharmaceuticals.
- Perform home health admission visits, re certification's, and discharges. (RN only)
- Record, assess and observe patient progress, reactions and symptoms.
- Assess the needs of patient and develop plan of care based on assessment.
- Check vital signs of patients, give injections, administer medication and wound care.
- Educate patients in line with physician recommendations.
- Document care in a patient record in accordance with agency, state and federal guidelines.
