Full-Time & Part-Time
Rehab Unit: Full-Time Opening
Day Shift: 6:00 AM – 2:00 PM
Evening Shift: 2:00 PM – 10:00 PM
Night Shift: 10:00 PM – 6:00 AM
Baylor Weekend Shift: 2:00 PM – 6:00 AM
Up to $5,000 Sign-On Bonus
Competitive pay with newly increased rates
$3.50/hour shift differential for eligible evening and night shifts

Provide high-quality nursing care in accordance with physician orders and individualized care plans
Administer medications, treatments, and nursing procedures safely and accurately
Monitor residents' conditions and promptly report changes to the RN, physician, or appropriate healthcare provider
Supervise and support Certified Nursing Assistants (CNAs)
Maintain accurate and timely electronic medical records (EMR)
Communicate effectively with residents, families, physicians, and interdisciplinary team members
Follow all infection prevention, safety, and regulatory guidelines to ensure quality resident care

Current, active Licensed Practical Nurse (LPN) license
Long-term care, skilled nursing, or rehabilitation experience preferred
Strong clinical assessment and communication skills
