We are seeking a Nurse Practitioner (NP) to deliver primary care to patients in skilled nursing facility settings. This role involves close collaboration with facility staff to ensure high-quality, on-site care. The successful candidate will foster relationships with facility staff and primary care/specialty providers in the community, provide education, and demonstrate clinical leadership. These efforts will directly address the complex needs of our patients and their families. This position offers flexibility and autonomy, with the ultimate goal of positively impacting the care of our older adult clients.
- Provide primary care for patients in assigned long term care facilities M-F 8AM- 3PM
- Work collaboratively with primary care/specialty providers to provide the best care possible
- Work collaboratively with facility staff and families
- Provide clear medical documentation of care provided, either written or within the EHR
- May be required to participate in on-call/after-hours call*
Qualifications and Education Requirements
- Certified Nurse Practitioner (ANP, GNP, FNP, AGNP) through a national board (ANCC, AANP)
- Minimum of master’s degree in Nursing
- Current unrestricted license (RN & NP) or the ability to obtain in state(s) in which practicing
- Unrestricted DEA (license to prescribe narcotics / level II info here) per state regulations (unless prohibited in state of practice) or the ability to obtain
- Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
Preferred Skills
- Long-term care experience in a variety of settings (SNF, ALF, group homes, CCRC, etc)
- Proficient with computer systems
- Understanding of geriatrics, chronic illness, and advanced/terminal illnesses.
Job Types: Full-time, Contract
Pay: $110,000.00 - $130,000.00 per year
