About the Role:
As a Registered Dietitian at Dry Harbor Nursing & Rehabilitation Center, you will play a vital role in promoting the health and wellness of our residents. You will work closely with our healthcare team to develop and implement personalized nutrition plans, ensuring that our residents receive the necessary support to achieve their dietary goals. With a focus on patient-centered care, you will provide education and support to residents, families, and caregivers, empowering them to make informed decisions about their health and nutrition.
Responsibilities:
Conduct nutritional assessments and develop individualized care plans for residents
Implement and monitor nutrition plans, making adjustments as needed to ensure optimal health and well-being
Collaborate with healthcare teams to provide comprehensive care and support
Educate residents, families, and caregivers on nutrition principles, meal planning, and cooking techniques
Participate in quality improvement initiatives to enhance the quality of care and services
Maintain accurate records and documentation, adhering to regulatory standards and guidelines
Requirements:
Must be a Registered Dietitian (RD) or eligible for licensure in the state of New York
Bachelor's or Master's degree in Nutrition or a related field
Strong understanding of nutrition principles and practices
