A field-based RN case manager position where you manage a caseload of hospice patients and coordinate care across settings. This position is exceedingly impactful on patients and families’ lives, and in turn will reward you with both professional and personal satisfaction.
This is not hospital bedside nursing – this is independent, relationship-driven clinical practice.

What You’ll Do
- Manage and coordinate care for hospice patients in the field (private homes, SNF facilities, AL facilities and the hospital)
- Provide care and support to both routine and GIP level patients
- Complete visits, assessments, and plan-of-care updates
- Collaborate with physicians and the interdisciplinary team
- Serve as a consistent clinical presence for patients and families
- Ensure timely, accurate documentation
Schedule & Work Hours
This is a full-time, 40-hour per week field-based position. The role includes a rotating weekend schedule as part of the standard clinical coverage model to ensure continuity of care for patients and families.

You are expected to perform—but you are fully supported in getting there and staying there.
What We Are Looking For
- Active RN license (NJ)
- Strong clinical judgement and communication skills
- Ability to work independently and comfortably in a field-based role
- Hospice/GIP experience preferred, not required

Pay: $95,000.00 - $105,000.00 per year
