Job Summary
Join our dedicated healthcare team as a Home Health Aide (HHA) - Personal Care Assistant (PCA) on a 12-hour shift! In this vital role, you will provide compassionate, patient-centered care to individuals in their homes or assisted living environments. Your energetic and proactive approach will help enhance the quality of life for those with diverse needs, including seniors, individuals with developmental disabilities, and those requiring memory or dementia care. Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices
- Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; document findings accurately in Electronic Health Record (EHR) systems
- Administer medications following medical documentation protocols and ensure compliance with HIPAA regulations to protect patient privacy
- Observe and report changes in patient condition, behavior, or safety concerns promptly to healthcare professionals
- Support patients with specialized needs such as catheter care, hospice care, dementia or Alzheimer's care, and memory care routines
- Assist with meal preparation and feeding to promote proper nutrition and hydration
- Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, and organizing patient spaces
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health settings is highly preferred
- Demonstrated ability to work with people with disabilities or developmental challenges
- Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation
- Certification in CPR is required; additional certifications such as medication administration, hospice care, or dementia training are advantageous
- Experience handling medical equipment such as Hoyer lifts or managing vital signs is desirable
- Strong communication skills to effectively collaborate with healthcare teams and families
- Ability to handle sensitive situations with compassion while maintaining professionalism
