The Family Counselor is a part of the multi-disciplinary team and is responsible for delivering intensive in-home and community-based services to 30 families with children in Scattered-Site Supportive Housing units. The Family Counselor provides therapeutic support, case coordination, and crisis intervention for families. The role focuses on improving family functioning, supporting mental and emotional well-being and promoting long-term housing stability. The Family Counselor works collaboratively with a multidisciplinary team to develop and implement service plans tailored to the unique needs of families and reports to the Assistant Director of Clinical Services.
Responsibilities
- Conducts regularly scheduled home visits to facilitate comprehensive needs assessments.
- Provide and/or coordinate linkages to health/ mental health care, parenting support, family activities, family reunification services, nutritional counseling and services, respite or alternative caregiver services, conflict resolution, mentoring, advocacy and other services as needed.
- Provides and/or coordinates linkages to parenting skills training such as first-time parenting programs, connection to prenatal services, OB/GYN and pediatrician for well-baby visits, mentoring, meeting evolving age appropriate milestones, family planning; access to childcare, after school programs, tutoring and summer camp services, avoiding gang involvement, and other skills to support parents to raising healthy families.
- Offers parenting support and psychoeducation to help caregivers strengthen parenting skills and improve family dynamics.
- Provides crisis intervention, suicide assessments, and safety planning as needed.
- Provides services using evidenced based and/or evidence- informed practices, including, but not limited to Motivational interviewing and person-centered planning, recovery- oriented and trauma informed case management.
- Completes person centered service plans focused on the strengths and needs of all the children in the household that may include physical and mental health, life skills, and developmental needs.
- Collaborates with the assigned Case Mangers, educational settings, child protective services, and other service providers as needed to support the families’ progress toward goals.
- Maintain accurate, timely and confidential documentation in accordance with agency and funding requirements.
- Attends staff meetings, inter- disciplinary meetings, trainings, and case conferences.
- Participates in special projects and performs other duties as assigned.
Qualifications:
- Master's Degree in one of the following fields: Marriage & Family Therapy, Social Work, Counseling r Clinical Mental Health Counseling, Psychology or Child Development.
- Experience 1-3 years of clinical experience working with children, youth and families.
- Experience with trauma-informed care, child development, or crisis intervention is preferred
- Completion of supervised fieldwork or internship in family or child counseling settings
- icensure or in progress in any of the following fields: (LMFT) Licensed Marriage and Family Therapist, (LCSW) Licensed Clinical Social Worker, (LPC/LMHC) Licensed Professional Counselor / Licensed Mental Health Counselor
