We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Personal Care Assistants (PCAs) to provide high-quality, person-centered care to individuals in their home setting. This role focuses on supporting daily living activities, ensuring patient safety, and promoting independence and overall well-being.
The ideal candidate has experience working with diverse populations, including seniors, individuals with developmental disabilities, and patients requiring specialized medical care. A strong understanding of patient care practices, medical terminology, and infection control standards is essential.
Responsibilities
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
- Safely use patient care equipment such as Hoyer lifts and transfer devices
- Monitor and observe changes in patient condition and promptly report findings to the Clinical Director
- Provide emotional support and companionship to enhance patient comfort and quality of life
- Follow infection control policies using aseptic techniques and proper hygiene practices
- Accurately document patient care and observations in EMR/EHR systems
- Maintain patient privacy and confidentiality in compliance with HIPAA regulations
Qualifications
- Prior experience providing care in home health, hospital, long-term care, or similar healthcare settings
- Knowledge of medical terminology, anatomy, physiology, and patient care procedures
- Experience working with specialized populations, including geriatrics, pediatrics, behavioral health, or individuals with developmental or physical disabilities
- Current CPR and Bloodborne Pathogen (BBP) certifications (required)
- Additional certifications such as Phlebotomy or Durable Medical Equipment (DME) experience are a plus
- Proficiency with EMR/EHR documentation systems
- Compassionate, dependable, and patient-focused with strong communication skills
- Ability to work independently and collaboratively within a multidisciplinary team
