Job Summary
Care Coordinators will work alongside clients to navigate a network of support on behalf of Opportunity House’s programs and services.
Responsibilities
- Conduct regular outreach to identify and connect with individuals experiencing homelessness.
- Provide information about available resources, including shelters, food services, healthcare facilities, and social services.
- Ensure individuals understand their rights and the voluntary nature of their participation.
- Maintain and document all interactions with clients in order to monitor progress in HMIS.
- Enroll and support clients with ECM and CS.
- Assist in triaging clients to the appropriate resources and services.
- Complete reports to track outreach outcomes and trends, contributing to program evaluation and improvement.
- Attend various meetings as a representative in the JPA, CoC, CES, HMIS, and Homeless Roundtable committees.
- Collaborate with community partners, local agencies, and service providers to enhance outreach efforts and expand resources.

Qualifications
Required:
● High school diploma or equivalency
● Previous experience in street outreach, social services, or community organizing
Desired:
● Lived experience and peer support specialist certification
● Bachelor’s Degree and 2+ years of Human Services experience
Job Type: Full-time
Pay: $25.00 - $31.00 per hour
Expected hours: No more than 40.0 per week
