Job Title: Home health RN - Registered Nurse
Locations: Newton, Fayville, Framingham, Hopkinton, Hudson - Massachusetts
Duration: 3-months contract (3-months extension)
Schedule: 40 hours day (8-hours x 5-days) - 8:00 AM-04:30 PM
Responsibilities:
- Assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
- Patient care is delivered within organizational philosophy, policy and standards of community health practice.
- Also required to perform admission processes and case management responsibilities policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Requirements:
- Must have an active RN license for the state of Massachusetts.
- 1 year of Certified Home Health Care Management experience.
- Staff worked with certified home health agency will be considered.
- Ability to travel to cases as assigned.
- Mileage $0.70 per mile provided.
