Job Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) to perform client assessments, reassessments, and supervisory visits for home care clients throughout Maryland. The RN will play an important role in ensuring clients receive safe, appropriate, and person-centered care while maintaining compliance with Maryland Department of Health (MDH) and regulatory requirements.
This is a great opportunity for an RN looking for flexible, per diem work with the ability to schedule visits around their availability.
Service Areas:
The RN will provide assessments and supervisory visits throughout Cecil County, Harford County, Baltimore County, and the Eastern Shore of Maryland.
Responsibilities:
- Conduct initial nursing assessments and reassessments for new and existing clients in their homes.
- Complete comprehensive evaluations of client needs, including medical history, functional abilities, medications, and safety concerns.
- Develop and update client care plans based on assessment findings.
- Perform supervisory visits to evaluate caregiver performance and client satisfaction.
- Monitor changes in clients’ conditions and communicate recommendations to the care team.
- Collaborate with caregivers, families, healthcare providers, and agency staff to ensure quality care delivery.
- Review documentation for accuracy and compliance.
- Provide clinical guidance and support to caregivers as needed.
- Maintain accurate and timely assessment documentation.
Qualifications:
- Current and unrestricted Registered Nurse (RN) license in the State of Maryland.
- Previous experience in home care, home health, geriatrics, case management, or community nursing preferred.
- Strong assessment, clinical judgment, and documentation skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and willingness to travel to client homes.
- Excellent communication and organizational skills.
