Family Living Caregiver / Provider (Host Home)Overview Join Lift Family Care as a Family Living Caregiver and play a vital role in helping an adult with intellectual and developmental disabilities live a supported, empowered life in a true family setting. Rather than an institutional shift-work model, this rewarding role involves welcoming an adult client into a nurturing home environment and providing the daily structure, companionship, and guidance they need to thrive.We are looking for dedicated individuals or families who want to share their lives and homes, helping our clients build pathways toward independence, community integration, and long-term well-being.What We Offer
- Compensation: $144.00 per day tax-exempt stipend (Difficulty of Care payment).
- Structured Commitment: This position requires you to be present and supporting the client for 12 hours per day (which includes overnight sleeping hours when the client is in the home).
- Comprehensive Support: Access to 24/7 on-call nursing, ongoing professional training, and monthly face-to-face visits from your regional Lift Family Care supervisor to assist you with resources and service coordination.
Duties & Responsibilities
- Daily Living Support: Assist the individual with daily routines, including healthy meal preparation (adhering to any prescribed nutritional or texture modifications), personal hygiene, dressing, and overseeing medication administration under the guidance of our agency nurse.
- Community Integration: Help the individual actively participate in the greater community to the same degree as individuals without disabilities. This includes driving them to local social clubs, public events, volunteer opportunities, or medical appointments.
- Employment Advocacy: Support the Employment First principle by assisting the individual in exploring, securing, and maintaining meaningful employment if they choose to work (including adjusting schedules to accommodate their shifts).
- Upholding Human Rights: Actively respect and protect the individual’s civil and human rights, including ensuring their bedroom privacy (lockable doors), facilitating unrestricted access to preferred food, and welcoming visitors of their choosing at any time.
- Environment & Safety Maintenance: Maintain a clean, safe, and hazard-free living space. Conduct routine safety checks and ensure life-safety equipment (such as operational smoke/CO detectors and fire extinguishers) are functioning.
- Care Documentation: Use the state-approved Therap and Electronic Visit Verification (EVV) mobile systems to log daily progress notes, track health milestones (like weight or medical appointments), and verify shift hours in real-time.
Job Requirements & Experience
- Home Suitability: Must own or rent a clean, safe house or apartment that can pass a comprehensive Family Living Home Study (including a private bedroom for the individual of at least 80 square feet with a latching door, proper heating/cooling, and water temperatures regulated between 100°F and 110°F).
- Background Clearances: Must successfully pass a Caregiver Criminal History Screening Program (CCHSP) fingerprint background check and an Employee Abuse Registry (EAR) clearance prior to placement.
- Driving Record: Must possess a valid Driver’s License, active auto insurance, a reliable vehicle, and have no DUI convictions or at-fault accidents within the previous two years.
- Certifications: Active First Aid and CPR certifications from a recognized organization (e.g., American Heart Association).
- Experience: Prior experience supporting adults with intellectual or developmental disabilities, or experience in caregiving, nursing, or human services roles, is highly preferred.
