Job Overview
Responsible for offering empathetic interventional, and transitional intensive case management services for any young adult 18-24 years old who may need help making the successful evolution from youth to being an independent adult. Through intesive case management and structured support, the Transition Specialist empowers young adults to find employment, education and housing opportunities, ultimately leading to independence. Facilitates individual, group meetings, and provides appropriate referrals to young adults as needed. Must have effective written and oral communication skills, organizational and leadership skills, ability to adapt to changes quickly and easily, and willingness to be a team player.
Duties include:
- Planning and scheduling independent living workshops for young adults
- Assisting young adults in accessing transition services and facilitating appropriate referrals to community-based services programs
- Serving as liaison between community and housing partners to monitor participant progress
- Attending/participating in team and community meetings
- Assisting participants in understanding and accessing services
Case management duties include:
- Plan and schedule in service training workshops for young adults
- Assist young adults access transition services.
- Serve as a liaison between community partners to monitor participant progress
- Facilitate appropriate referrals to community-based programs.
- Assist staff in interpreting assessment results and recommending appropriate services.
- Assist vocational partners in adapting curriculum
