ABOUT THE ROLE
HouseCall MD is seeking a Nurse Practitioner to deliver high-quality, patient-centered care in home and facility settings across the Santa Clarita area. You'll have the clinical autonomy to practice at the top of your license while being supported by a strong administrative and clinical team, so your focus stays where it matters most: the patient.
No formal on-call. No volume-driven model. Just meaningful, relationship-based care.
WHAT YOU'LL DO
- Provide comprehensive primary care to patients in their homes and care facilities
- Develop and implement personalized treatment plans
- Review and utilize medical records to guide clinical decisions
- Communicate clearly with patients, families, and care teams
- Educate patients on their conditions, treatments, and care goals
- Deliver compassionate, holistic care aligned with each patient's needs

QUALIFICATIONS
Required:
- Active Nurse Practitioner license (California)
- Minimum 2 years of primary care experience
- Strong clinical knowledge and sound decision-making skills
- Excellent communication and interpersonal skills
Preferred:
- Experience in home health, geriatrics, or palliative care
- Proven ability to build long-term relationships with patients and families
SCHEDULE
- Flexible hours, typically between 7AM and 6PM
- No formal on-call requirement
- Occasional urgent patient-related communication as needed
- In-person, home visits and facility-based care throughout Santa Clarita, CA
