Position Summary
As an RN Home Health, you will provide skilled nursing care to patients in their homes while coordinating their overall plan of care. You will serve as an important member of the interdisciplinary care team and help patients remain safely and comfortably in their homes.
Responsibilities
- Conduct comprehensive nursing assessments during home visits
- Develop, implement, and update individualized patient care plans
- Provide skilled nursing care in accordance with physician orders and applicable regulations
- Monitor patient conditions and identify changes requiring intervention
- Coordinate care with physicians, therapists, aides, social workers, and other members of the healthcare team
- Educate patients and caregivers regarding medications, disease management, treatment plans, and preventative care
- Complete accurate and timely clinical documentation
- Communicate effectively with patients, families, physicians, and the interdisciplinary team
- Manage an assigned caseload while maintaining high standards of patient care
- Help patients and families navigate their healthcare needs and achieve their goals
Qualifications
- Current, unrestricted RN license in the State of Connecticut
- Previous home health experience strongly preferred
- Strong clinical assessment and critical-thinking skills
- Excellent communication and organizational abilities
- Ability to work independently while collaborating effectively with a team
- Reliable transportation and willingness to travel to patient homes throughout the Stratford area
- Commitment to providing compassionate, patient-centered care
