Weekend availability of at least one weekend per month is required.
Per Visit Rates
- Admission: $100
- Recertification / Resumption of Care (ROC): $75
- Revisit: $62
What You'll Do
- Perform comprehensive patient assessments at the start of care and throughout treatment.
- Develop and coordinate individualized plans of care.
- Provide skilled nursing care and treatments as ordered.
- Monitor patient response to treatment and provide education to patients and caregivers.
- Coordinate care with physicians and interdisciplinary team members.
- Refer patients to additional services and resources as appropriate.
- Develop discharge plans and ensure continuity of care.
- Document all patient care activities in the electronic health record.
Qualifications
- Current New Jersey Registered Nurse (RN) License
- Valid Driver's License and reliable transportation
- One year of home health experience preferred
- Strong assessment, communication, and organizational skills
- Ability to work independently while providing exceptional patient care
