Duties
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts.
- Monitor patient health by observing vital signs and reporting changes promptly to nursing staff.
- Support medication administration following proper protocols and documentation standards while maintaining compliance with HIPAA regulations.
- Provide specialized care for residents with dementia, Alzheimer’s disease, or other memory care needs, ensuring their safety and comfort.
- Assist with medical procedures such as catheter care, wound care, and hospice support as directed by healthcare professionals.
- Maintain accurate medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring thorough documentation of patient care activities.
- Support household tasks including cleaning, laundry, meal preparation, and maintaining a safe environment to promote overall well-being.
- Work collaboratively within a team to deliver home & community-based care that respects each individual’s dignity and preferences.
- Engage in patient observation to identify potential health issues early and communicate findings effectively to the healthcare team.
- Follow all safety protocols during patient handling and use of equipment like the Hoyer lift or mobility aids.
- Assist with hospice care when needed, providing comfort and emotional support to patients and their families.
Experience
- Prior experience working in nursing homes, assisted living facilities, or home health environments is highly preferred.
- Demonstrated ability to work with individuals with developmental disabilities or dementia-related conditions such as Alzheimer’s disease.
- Knowledge of medical terminology and familiarity with medical documentation practices is essential.
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or catheter care are advantageous.
- Experience using EMR/EHR systems for documentation and patient record management is desirable.
- Strong skills in patient observation, caregiving, and home & community care are necessary to succeed in this role.
- Ability to handle physically demanding tasks safely, including lifting patients with equipment like Hoyer lifts or performing transfers.
