Join our dedicated pediatric home care team as a Pediatric Home Care Nurse within the GAPP (Geriatric and Pediatric Advanced Practice Program).
In this vital role, you will provide compassionate, comprehensive nursing care to pediatric patients in their homes, ensuring they receive the highest quality of medical attention and support. You will utilize your extensive clinical skills, including critical care, ICU, and emergency medicine experience, to assess patient conditions, coordinate care plans, and deliver personalized nursing interventions. Your expertise in pediatrics, medical-surgical nursing, and post-acute care will be essential in promoting optimal health outcomes and enhancing patient comfort. Responsibilities
- Conduct thorough patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine individualized care needs.
- Administer medications, treatments, and interventions as prescribed by physicians, ensuring adherence to safety protocols and HIPAA regulations.
- Provide education and support to families and caregivers on disease management, medication administration, discharge planning, and emergency procedures.
- Monitor patient progress through diagnostic evaluation and ongoing assessment; adjust care strategies accordingly to optimize health outcomes..
- Coordinate outpatient visits, clinic appointments, and post-acute care services with families.
- Qualifications
- Proven critical care experience with a strong background in ICU or emergency medicine settings; experience in Level I Trauma Centers is highly desirable.
- Extensive pediatric nursing expertise including PICU (Pediatric Intensive Care Unit), inpatient, outpatient, or clinic experience.
- Knowledge of infectious disease care, behavioral health considerations, geriatrics (if applicable), and hospital medicine practices.
- Familiarity with medical terminology, physiology knowledge, discharge planning processes, and diagnostic evaluation techniques.
- Strong skills in patient assessment, vital signs monitoring, case management, and utilization management within acute or post-acute settings.
- Experience working with managed care organizations and understanding of HIPAA compliance for patient privacy.
- Ability to work independently in home environments while maintaining high standards of patient service and safety.
Pay: $35.00 - $55.00 per hour
